Summary of Complaint Investigation Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and complaint investigation. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. Agency had an instance of vaping inside the building. It is observed that an educator leaves the classroom often, leaving only one staff member with the children. During complaint investigation, video footage was observed showing children left without direct supervision on the playground. Children have been withheld from outdoor activity time for a small portion of time for behavior. Agency was observed not following all CACFP nutrition guidelines.

See Below for Detailed Information Observation: Child left gym while door was open and went up on walking track above gym. Staff were unaware that he was missing until they saw him hanging from rail. 1240-04-01-.11 (1) (c) 1. Compliance Statement: Each teacher involved in the incident has been coached by the interim director in supervision strategies and center policies. The child’s teacher has met with his parents and offered apologies. The church has locked the walking track and walkers must request entrance at the church office. Procedures for supervision in the larger motor areas have been improved. Observation: Agency did not document incident and provide to parent on the same day of incident. 1240-04-01-.09 (1) (e) Compliance Statement: The board of directors asked for the resignation of both co-directors who did not follow DS regulations or center policy. The board is taking steps to be sure center policies align with DHS regulations and will train new director in those regulations and policies. The board also asked for the resignation of other co-director when it was found that she was aware on day 2 and did not document or provide parents with information. Agency will comply with DHS regulations and provide reporting to parents on same day of incident. Observation: Agency did not let parent know on same day of incident that occurred with Max on 01/11/2022. 1240 – 04 – 01 - .09 (1) (a) Compliance Statement: Verbal and written training for all staff was done on the important of communication with parents. Use of “ouch reports” and verbal communication was stressed. Interim Director is aware of regulations and will ensure that new director is acquainted with all DHS regulations and knows there to obtain further training. Observation: Agency did not notify DHS of incident that occurred with child on day of incident. 1240 – 04 – 01 - .09 (1) (f) Compliance Statement: The board of directors asked for the resignation of both co-directors who did not follow DS regulations or center policy. The board is taking steps to be sure center policies align with DHS regulations and will train new director in those regulations and policies.
