Summary of Complaint/Probation Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and complaint and probation visit. The agency is transitioning and shuffling children due to being short-staffed. Parents nor the department were notified on the day the incident occured. Three (3) children transitioned from the 6 weeks- 15 month old classroom to the one-year old classroom at 4 o'clock P.M. Educator was observed raising her hand as though she was going to strike a 3 year old child. The agency did not immediately report the suspicion of child abuse/neglect to the Department of Children's Services.

An educator was heard calling a child out of their name, using profanity. An educator has not had a physical after 10 days of hire date. An educator was observed popping a child on the hand.

On 12/14/25 agency ratio in the 2-3 year old room was 1:14. Per the state Licensing Rules and Regulations the ratio for this age group is 1:8. During visit to agency staff attendance records were requested by the Department, but not made readily available It was found that the agency does not have daily defined groupings per classroom.

During an incident that occurred at the agency, it was found that a child was left unattended by an educator. It was found that during an incident at the agency a pre-school child was isolated for behavior management. Incident Report was not completed immediately upon incident nor was a copy provided to the parent on the same day. The incident was also not reported to DHS or DCS on the same day of occurrence. After an incident occurred at the agency, the agency conducted its own investigation without reporting to DHS or DCS. It was found during unannounced visit with DHS that an incident occurred that including inappropriate discipline of a 4 year old.

Incident reported that a child received a gash on their ear while playing in the block center.

During observation, LC Bruce found the infant room was not in ratio. Incident reported that child was left unattended on playground during transition to classroom from playground.
