See Below for Detailed Information Observation: : LC determined through verbal reports and paper roll sheets, that the educator in charge of the children did not follow through and do name to face roll after each transition area upon returning to the classroom after outside play. Thus, resulting in a child remaining outside while the class was inside the building. Rule: 1240-04-01-.11 (3) 6 Corrective Action: CCRR will come to do 10 hours of Conscious Discipline training with the staff. This training will be mandatory. This will ensure we are all well versed on developmentally appropriate behavioral interventions, appropriate and reasonable discipline children can understand and new techniques for disciplining. After training is complete, there will be documentation in each staff member's personal file documenting their completion of this course and training. Failure to comply with this training or to implement will lead to disciplinary action including possible termination. + I will have my staff commit to partnering with me and Kindercare's Inclusion's department if they are experiencing difficulty with handling a child's behaviors. This will be documented in the staff member's personal file as well. + There will be walkie-talkies in every classrooms. If a teacher needs assistance or a break, they can utilize the walkie-talkie to speak with another adult or ask for assistance if they are overwhelmed. +Center management will be conducting weekly classroom observations (minimum of 2 classrooms per week) to coach teachers on any actionable items to include proper child guidance and discipline practices. Observation: After talking with the director and staff concerning a child being left on the playground, LC determined that there was enough evidence to support the fact that a child was left on the playground for a period of 1-2 minutes unsupervised. Rule:1240-04-01-.11 (1)(a) Correction Action: + The entire staff will receive training from CCRR to be well-versed in transitions and routines and engaging name-to-face transitions and interactions with children. +Center management will be conducting weekly classroom observations to coach teachers in-the-moment on any actionable items including developmentally appropriate practices and to meet each child's needs for where they are at. These observations will be documented and will be placed in the teacher's being observed personal file. +Center management will determine when/if a child's guardian should need intervention in the form of a conference or possible evaluation from an outside source (TEIS, ABA, Inclusions thru Kindercare, CCRR assistance and training, etc.) and be responsible for setting up the conference with inclusion of the teacher. Observation: LC conducted an investigation for two complaints of alleged abuse by an educator. After interviewing the staff, it was determined that the staff knew about the allegation on 3/15/22 but did not report it until 3/23/22. Rule: 1240-04-01-.10 (1)(a) 1 Correction Action: +I will re-train the staff on their individual responsibility and requirement to immediately report any reasonable suspicion of child abuse or neglect to either DCS and/or local law enforcement or the judge of the juvenile court in the county of the child's residence by ensuring the phone numbers are posted in every classroom and the staff bathroom as well as the kitchen and in the lobby. + I will enforce each staff member has a signed document in their personal file stating their understanding and commitment of their role as a mandated reporter. After training is complete with both of these items, there will be documentation in each staff member's personal file documenting their completion of this course and training. Failure to comply with this training or to implement will lead to disciplinary action including possible termination. Observation: Multiple statements were given that concluded that the educator yelled at a child and used curse words because he soiled in his diaper. The child was shamed and reprimanded in front of his classmates. Rule: 1240-04-01-.15 (5)(a)(b)(c) Corrective Action: + All center staff will be receiving training Tuesday, March 29th from CCRR in regards to transitions and going over the CSR (Child Supervision Record). This training will highlight calling roll and utilizing the name-to-face feature on our CSR as well as the name-to-face page specifically for transitions such as, but not limited to, arrival and leaving the playground. -In addition to this training, I will also be going over engaged transitions that involve proper name-to-face and engage teachers with children to ensure maximum supervision during high-risk times. +I will be adjusting the playground schedule so classrooms are not outside while combined at the beginning or end 1.5 hours of the day to reduce high-risk supervision time. +Center management will be checking off on CSRs in every classroom, every day and signing off. Center management will be observing engaging name-to-face transitions to and from the playground weekly. + There will be documentation is each person's file to show they have received adequate training. Failure to comply with this training will result in disciplinary action including possible termination.
