The agency is going to send PE a plan of action for updating/cleaning the rooms in the agency. Teachers have already been moved and steps have already begun in this process. Observation: While viewing a video of a classroom for a complaint investigation, the teacher was changing children in front of other children in a "non-private" location. Children were fully exposed to other children. The child would just stand in front of the teacher and the teacher would pull off the pull-up and put another one on while the child stood exposed in front of other children. Violation Type: 1240-04-01-.12 - Health and Safety Plan of Corrective Action: The agency will work with CCR&R to create a more "private" area for children to be changed while considering the fragility of some of the children and their fears of being alone in a closed room.

(June 28th video) Plan of corrective action: The agency states that the said teacher was given popsicles by a parent and improperly used them as a tool for good behavior. this teacher and entire staff have been instructed to never use food to enforce discipline or to reward good behavior. If food is given as an extra treat, all children will receive the treat no matter what the behavior has been. Supervision, proper discipline, behavior management and classroom managements trainings are scheduled for the agency staff Violation 1240-04-01-.15 (5c) – Program Observation: An educator shamed a student for “not doing what she should have done” after being caught in an inappropriate situation. Children were also yelled at numerous times by the same educator. (June 28th video 12:48) Plan of corrective action: The agency states that the teacher has been working with this student and parent re: inappropriate behaviors and was observed asking the student why said student had not followed the plan. Said teacher was shown video of the incident, reports understanding the infraction of the wording used with said student, and was placed on a 90-day probation period. Random classroom monitoring will take place. Supervision, behavior management, proper discipline and classroom management training have been put in place for the entire staff. Violation 1240-04-01-.11 – Supervision (1a) Observation: The educator in the classroom was not aware of the activities of several children for five or more minutes. (June 28th video footage 12:41 – 12:47) Plan of corrective action: the agency states that the teacher was observed cleaning up the lunch area in the classroom and did not see some of the behaviors in the classroom during that time. This teacher was shown the video, reports understanding the infraction of not being aware of inappropriate behaviors of children as they happened due to multi-tasking/inability to see all children at one time, and was placed on a 90-day probation. This safety plan will be lifted upon completion of trainings and coaching. Said teacher will go through the trainings with the entire staff and has been placed on a safety plan as well. Safety plan will be lifted after completion of training and random classroom monitoring will take place. The agency states that while understanding the severity of the violation that no child was hurt and that the director was unaware that the way the child was picked up was a program issue.

Observation: During complaint investigation on 5/22/18, it was verified that a caregiver did not strap an infant in a high chair which resulted in the child falling out and hitting his head. Two other staff knew of the incident as soon as it occurred, yet did not inform management staff regarding it. Observation: During complaint investigation on 5/22/18, it was verified a child fell out of a high chair hitting his head. The child was not strapped into the high chair as required. 1240-04-03-.08 Equipment for Children Corrective Action: Agency states that all staff will be trained in proper use of equipment and equipment standards. Management will also inspect to ensure that equipment is in proper working order. Observation: During complaint investigation on 5/22/18, it was verified that a caregiver did not strap an infant in a high chair which resulted in the child falling out and hitting his head. Observation: During complaint investigation on 5/22/18, FS1 Hunter observed a caregiver pick a toddler up by his arm rather than his body. 1240-04-03-.07 Staff Corrective Action: Agency states that all staff at the center will receive training on proper handling of children. Observation: During complaint investigation on 5/22/18, it was verified a child fell out of a high chair hitting his head. No accident/injury report was filled out by the caregiver until the director mandated that she do so later that day. 1240-04-03-.05 Ownership, Organization, and Administration Corrective Action: Agency states that all staff at the center will receive training on the proper way to address incidents and accidents when they happen at the center. It will be clear that all incidents should be recorded and reported to management.
