See Below for Detailed Information Observation of violation: After speaking with the agency owner, agency director and an educator for the two-year-old classroom, it was determined that a child had exited the agency unattended. Violation: 1240-04-01-.11 – Supervision Plan of Corrective Action: The agency owner will put “alarm” type instruments on each of the doors of the classrooms leading outside of the agency. The owner will also work with CCR&R to receive training on “runners” and other types of supervision.

See Below for Detailed Information 1240-04-01-.11 – Supervision (6a9) Observation: Three sleeping infants were not checked after 15 minutes. 1240-04-01-.07 – Criminal Background Checks Observation: There were staff members on site at the agency who did not have required background checks/OK to work letters. Staff was made aware that they could not return to the agency until fingerprints had cleared and an OK to work letter was received. 1240-04-01-.11 – Supervision (1a). Observation: Incident forms showed that the injuries/incidents were occurring on the playground area frequently and that supervision in that area could be improved. Three incidents of biting involving the same two children have occurred in this area in a short amount of time showing inadequate supervision being provided.

See Below for Detailed Information Observation: During complaint investigation on 12/21/17 it was discovered that staff person Simone Rain (started 9/13/17) did not have a physical on file as required. Corrective Action: Agency states that moving forward they will make sure all teachers have a current physical in their file. 1240-04-03-.05 Ownership, Organization, and Administration Observation: During complaint investigation on 12/21/17 it was discovered that complete staff files were not maintained for at least one year following separation. Corrective Action: Agency states that moving forward they will make sure all staff files are maintained on site for a t least one year after separation. 1240-04-03-.05 Ownership, Organization, and Administration Observation: During complaint investigation on 12/21/17 it was discovered that the playground gate between the toddler playground and the pre-k playground has an opening / gap due to a bent pole. Corrective Action: Agency states that webbing will be added to cover the gap / opening until the pole and / or gate can be replaced. 1240-04-03-.08 Equipment for children Observation: During complaint investigation on 12/21/17 it was discovered that one staff had not completed the shaken baby training as required. Corrective Action: Agency states that moving forward they will make sure all teachers have the Shaken baby training in their file before they begin working. 1240-04-03-.07 Staff
