Summary of Safety Plan Monitoring and Complaint Investigation Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and Safety Plan Monitoring, and Complaint Investigation. A three-year-old child was placed in a high chair to keep the child from running out of the building. A child's file was requested to be reviewed by the LC. The child 's file was not present on-site.

The infant was observed sleeping in a rocker seat. Two infants were present, with one pack-and-play observed. The educator reported that they did not have cribs or pack and plays for infants to sleep in. There were bed pillows in the pack and play instead of the orignal mattress that came with the pack and play. The infant was asleep while wearing a bib. Technical assistance was provided.
Six children, including infants and two-year-olds, were observed napping in the six week to thirty month old classroom. This classroom was out of ratio due to exceeding the one adult to children ratio for this classroom. Technical assistance was provided.
