See Below for Detailed Information Observation: During the annual assessment, there was a lack of resilient surfacing noted on the playground. 1240-04-01-.14 Equipment for Children Corrective Action: There shall be a minimum of six inches of resilient surfacing for playground equipment that takes a child's feet off the ground. Observation: During the annual assessment, diaper checks were not performed as required. 1240-04-01-.12 Health and Safety Corrective Action: Agency will perform diaper checks as often as necessary, but at least every two hours. Observation: During the annual assessment, the agency did not perform diaper changing sanitation or hand washing procedures as required. 1240-04-01-.12 Health and Safety Corrective Action: Diaper area sanitation and hand-washing procedures shall be performed as required. Observation: During the annual assessment, the agency did not sanitize mealtime tables as required. 1240-04-01-.13 Food and Food Service Corrective Action: Meal tables shall be cleaned and sanitized as required prior to and after meal service. Observation: During the annual assessment, a hazardous item was observed to be left in reach of the children. 1240-04-01.12 Health and Safety Corrective Action: All items which pose a hazard to children shall be out of reach to them at all times.

See Below for Detailed Information Observation: Blankets were observed in pack n plays in the infant room. 1240-04-01-.11 Supervision Corrective Action: Blankets were removed immediately; Agency will retrain infant room staff on safe sleep requirements. Observation: A caregiver was observed standing outside the playground area on her phone instead of supervising the children who were playing. 1240-04-01-.11 Supervision Corrective Action: The agency will retrain staff on the playground supervision plan. Observation: A caregiver engaged in a verbal altercation with a parent on the premises. 1240-04-01-.06 Staff Corrective Action: The staff member will be terminated, and the agency will retrain staff on appropriate parent interactions.

Compliance statement: To be in compliance, all items labeled "keep out of reach of children" shall be stored so that the children cannot access them. Observation: During the agency's annual assessment, proper diapering procedures were not followed in the infant room. 1240-04-01-.12 (15) Diapering. (g) Educators shall utilize sanitary diapering procedures: 3. Diapering surface shall be washed with soap and water and sanitized after diapering each child; Compliance statement: To be in compliance, the diapering surface shall be washed with soap and water and sanitized after diapering each child. Observation: During the agency's annual assessment, proper handwashing procedures were not followed for children or staff at mealtime. 1240-04-01-.13 (1) Meal and Snack Time Procedures. (a) Staff shall wash their hands with soap and water before preparing and serving meals and after meals and snacks. Children shall wash their hands with soap and water before and after meals and snacks. Compliance statement: To be in compliance, staff shall wash their hands before and after preparing and serving meals; children shall wash their hands before and after meals and snacks. Observation: During the agency's annual assessment, infants were observed sleeping in rockers. 1240-04-01-.11 SUPERVISION (6) Safe Sleep Supervision Procedures. (a) Because of the possibility of Sudden Infant Death Syndrome (SIDS) and to prevent suffocation deaths in infants: 1. Infants shall sleep in cribs or play yards; 2. No infant shall be allowed to sleep on a sofa, soft mattress, adult bed, in a car seat, in a swing, or in other restraining devices; Compliance statement: To be in compliance, infants must be placed in a crib or play yard to sleep.
