Technical Assistance Provided: 1. Supervision retraining is required for all staff. 2. Three additional unplanned fire drills need to be conducted each month during February - April of 2023. Staff need to be thoroughly retrained in supervision as soon as possible. 2. No Violations were received OR Violations were received and include: 1. Supervision 2. Physical Facility 1240-04-01-.16 PHYSICAL FACILITIES. (c) The areas where children play or are cared for shall be properly maintained: 1. A pre-play/care inspection of the outdoor play area shall be completed by the child care agency prior to each use by children. 2. The play/care areas shall be free of hazardous conditions, items, or materials. 3. 1240-04-01-.15 PROGRAM, LANGUAGE, AND LITERACY DEVELOPMENT Areas where a child sleeps shall have adequate lighting which allows the educator to see each child with a quick glance and respond appropriately to each child's physical and emotional needs. Classrooom 3 was too dark to see a child who might need help. Correct Action Plan (CAP) completed. A four year old child left his classroom unobserved. The classroom educators were unaware he had left until another adult who happened to be walking through the hallway brought him back to his classroom. An adult ladder in the hallway, extending past the ceiling is accessible to children. The lighting in classroom 3 is not adequate for an educator to see if a child has physical or emotional needs.

The child was lying down in a side area of the classroom and she did not realize that Walden was not with the group. She took three of the four children present to the playground. Once on the playground, she checked her roster and realized he was missing. Another staff member went back into the building for him and brought him to the playground with the other children for the duration of the drill.
