On 5/19/26, LC Chastain and FS Pass conducted a complaint investigation. During the visit they observed the educator take four children inside from the playground. The educator did not do face-to-name roll call during the transition.

Rule 1240-04-01-.06, (g) required trainings- • One staff members did not have documentation of required trainings in their staff file or on the TNPAL website. • On 4/4/22 and 4/14/22, opening staff member did not have a diploma. Rule 1240-04-01-.06 (3) (h) (1) (2) Jazziona Harris did not have the required trainings completed before working with children. Rule 1240-04-01-.08, Record Keeping- (4) (6) (11) • One staff member does not have references, or a physical on file. • Missing staff file documentation and falsified documentation. • One part time staff member did not have a file on site. • One employee marked that she graduated high school, but did not have a diploma on file. Rule 1240-04-01-.05, (15) Falsification of any information or documentation is prohibited. • High School diploma- two did not match the application and were similar templates. One of employees with a computer generated diploma template stated that they did not graduate high school on their application. The other employee with a computer generated diploma template listed a high school that did not match the diploma. • Employees references appeared to be duplicated. Exact same hand writing and wording for all three references. On two references, the first name had been scribbled and another name added.
