Summary of Compliant Investigation Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and complaint. 12040-04-01.11 Supervision (3) 1240-04-01.10 Duty to Reprot Child Abuse LC observed the infant 1 classroom had one educator with five children ages 6wks. to 15mos. The ratio should have been one educator to four children. A complaint came in that a child was left unsupervisied for less than a minute. LC observed an educator was not directly supervising children ages 24 to 35mos. during meal time. Duirng an unnaounced visit it was dicovered that the director did not report to Department of Children's Service and Department of Human Services that a child was left unattended. A complaint came in that a child was left in a classroom for less than a minute unsupervised. The lapse in supervision was not report to the Department of Children's Services.
