See Below for Detailed Information Observation: During an unannounced monitoring visit to investigate a compliant, PE found the agency had not reported two incidents that had occurred to the TDHS Child Care Compliant Hotline. 1240-04-01-.09 INCIDENT REPORTING Plan of Corrective Action: The agency will ensure any incident, illness, or injury will be reported correctly to the Child Care Compliant Hotline. Observation: During an unannounced monitoring visit to investigate a complaint, PE found that a child had been left on the playground for an unknown amount of time. The activities and whereabouts of the child were unknown. Direct supervision was not maintained. 1240-04-01-.02 DEFINITIONS Plan of Corrective Action: The agency will ensure direct supervision is maintained at all times. The activities and whereabouts of the children will be known. Observation: During an unannounced monitoring visit to investigate a complaint, PE found one serious incident to have occurred at the agency. One child being left on the playground for an unknown amount of time. There was no report documented of this incident occurring. 1240-04-01-.09 INCIDENT REPORTING Plan of Corrective Action: The agency will ensure all serious incidents will be documented and reported to the parent/guardians. Observation: During an unannounced monitoring visit to investigate a complaint, PE found that a child had been left on the playground for an unknown amount of time. The whereabouts and activities of the child were unknown. Direct supervision of the child was not maintained. 1240-04-01-.11 SUPERVISION Plan of Corrective Action: The agency will ensure direct supervision for each child enrolled is maintained at all times. Observation: During an unannounced monitoring visit to investigate a complaint, PE found an educator had not conducted a visual inspection of the classroom and playground to ensure all children had been released to their parent/guardian. 1240-04-01-0.11 SUPERVISION Plan of Corrective Action: The agency will ensure visual inspections are conducted thoroughly at the close of business each day.
