Children's play equipment was observed in the hallway where children walk, creating a tripping hazard and obstructing a clear path of travel. The program must ensure that hallways and walkways remain free of obstructions at all times. Broken playground equipment was observed on the outdoor playground presenting a safety hazard to children. A bottle of Clorox and water solution used for sanitizing was placed on top of a shelf within reach of children.

A 3 year old child was left unsupervised in the hallway after returning from the outdoor playground. An educator improperly disciplined a toddler by picking him up by one arm and moving him across the room. Educators failed to conduct roll call when returning back to the classroom from the outdoor playground.
