Summary of Complaint Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and investigating a complaint. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. During an unannounced visit to investigate a complaint on 03/11/2025, video footage from 02/18/2025 of the Adventures 1 classroom (ages 1-2 years old) where the ratio was 1:8. The required ratio for that age group is 1:7. During an unannounced visit to the agency on 03/11/2025, to investigate a complaint, video footage was reviewed and it was seen that children were sitting at the table for several minutes when the educator was observed looking in her phone instead of closely watching the children when an incident was observed where a child bit another child. During an unannounced visit on 03/11/2025, to investigate a complaint, the agency's licensing consultant and Field Supervisor observed that the agency did not report an incident where a child bit another child's finger (removing some of the skin) which occurred on 3/3/2025. The childcare agency should have notified the Department on the same day of the incident.

The Licensing Consultant made an unannounced visit to the agency and observed one educator present in the large eating area with nine children (ages 1-4 years old). One-year-olds should not be combined with preschool children. Also, there were 6 children (ages 4 months to 1 yrs old) in the school-age classroom with one educator (1:6) Individual parent meetings to discuss the child's progress and development was not maintained in the child's record. There was no documentation of the agency reviewing the transition plan with the family for infant, toddler or pre-school children from one age group to another. Agency is not currently accredited. Educators should talk with children through serve-and-return conversations, noticing all nonverbal cues, including eye contact, gestures, touch, smiles, pointing, etc. Educators should also ask questions to build children's dialogue. Educators should integrate new words during shared reading and play. Instances of these things did not occur. The educator should demonstrate knowledge of the following in their daily educational activities to enhance the learning environment. This was not observed and the lesson plan did not reflect the activities observed by the licensing consultant. Educators should join in and play while using serve-and-return interactions to build connections with children and support conversational turns (not observed). During an unannounced complaint visit on 2/7/2025, the Licensing Consultant observed one educator present in the large eating area with nine children upon arrival and another child arrived during the walkthrough, which made it 10 children (ages 1-4 years old). One-year-olds should not be combined with preschool children. Also, there were 6 children (ages 4 months to 1 yrs old) in the school-age classroom with one educator (1:6), which made the ratio out of compliance for both groups. During an unannounced complaint visit on 2/7/2025, the Licensing Consultant (LC) observed 5 children (ages 4 months to 1 yrs old) in the school-age classroom unattended. Two of the one-year-old children were drinking milk from their bottles. There was no educator present in the room when the LC entered the room. During an unannounced visit, the Licensing Consultant observed one educator hired in January, did not have a physical(medical clearance) on file. During an unannounced complaint visit on 2/7/2025, the Licensing Consultant (LC) entered the School-Age classroom where there were 5 infants present (ages 4 months to 1 yrs old) unattended. There were 4 children in high chairs. Two of those children (ages 1) in high chairs were drinking milk in their bottles (they were not supervised). The LC told the educator that the younger children were unattended. An educator came out of the kitchen and into the room saying "I'm right here". The LC told her that the children were unattended when the LC entered the doorway.
