A child is prescribed two medications, both of which are onsite. Program has only obtained signed authorization to administer one of the meds. A physical in a child's file listed medications for allergy/asthma related symptoms. The program does not have the medications onsite and/or any of the required documentation. It is unclear if the child still needs the medications or were prescribed temporarily.
Staff files have evidence of being trained/certified in Pediatric CPR via an online training. Files did not contain evidence of the initial certification being done in person.
In the Toddler 2 room, the area above the diaper changing table contained several items which could potentially fall onto a child during a diaper change. Two toilets in the Toddler 1 room are not working.

The Young Toddlers Room was out of ratio twice due to one of the staff members leaving the room.
