Licensor had to remind provider of the child who was climbing on the couch. Provider stated this child is autistic and usually keeps to himself. Licensor observed the child secluding himself and had a phone throughout this visit.

Licensor observed children's files have not been updated since the child had been enrolled in program (2023-2024).
Licensor reviewed three children files. Of the children reviewed all were missing a photo consent form and one child was missing the permission to post allergies (for Lactose allergy). Per provider, the mother has stated the child no longer has a lactose allergy.
Licensor observed fire inspection to be expired on 11/29/2025. Licensor reminded provider the fire inspection needed to be completed annually. Provided had not scheduled fire inspection since 11/29/2024.
Provider only completed 1.5 hours of Professional development hours during 2025 (according to RISES), and her assistant had no hours completed in 2025 (according to RISES).
Licensor observed a child enrolled in program had lactose allergy and Allergy Information Form was not in file.
Licensor observed provider no cleaning diaper-changing surface after each use. Licensor reminded provider of this regulation.
