Provider has two children enrolled in program. Licensor reviewed both children's file. One child started last week. This child moved to RI from another country and does not have a pediatrician. Parents are in the process of obtaining health insurance to then find a pediatrician for the child. Parents do not have physical/immunization records from previous pediatrician. This child's Emergency Medical treatment form was not complete, no photo consent, no full enrollment packet (that includes additional Infant/toddler file requirements), topical cream consent. The other child did not have full enrollment packet (that includes additional Infant/toddler file requirements), physical, immunizations, emergency medical treatment form, Emergency pick up, photo consent, activity consent, topical cream consent.
