Licensor reviewed children files. It was observed that Infant was missing evidence of physical.
Assistants are missing CPR and not uploaded on RISES. Licensor did not assess these during the visit as the assist had reported they had already uploaded it in RISEs.
Assistant reported they do not have a bottle warmer and no children that is currently enrolled drinks their bottle warmed.
Licensor observed files not to be updated yearly. Licensor reminded assistant of this regulation.

Licensor observed a screen door, tire, counter tops, broom and other items on the back fence that needs to be removed.
Licensor reviewed children’s files enrolled in program. Infant was missing evidence of a physical.
Licensor observed assistant left 3 month old infant carrier out of sight and sound supervision while she went to change other child diaper in the next room.
Licensor observed children spread out throughout two room (kitchen area/office space) in the program where sight and sound was challenging to maintain.
Provider was not on site during this visit. Provider reports she was at an appointment. However, her assistant was onsite with the children (3).
Licensor observe formula not to be labeled with child's name.
Licensor observed assistant to use gloves to change infant/toddler's diaper and change the gloves in between child. However, assistant did not wash her hands in between changing diapers.
Licensor observed assistance changing infant/toddler’s diaper. A disposable changing paper liner was placed over the changing table and disposed of once finished changing the infant/toddler however changing tables was not sanitized before or after.
