Licensor observed children files to be missing immunization records.
Licensor was unable to review required documents/inspections as provider has not upload the documents. Provider reported she did have the inspections updated however did not have them printed at the time of this visit.

Licensor reviewed children files enrolled in program. Many children were missing physicals (or had expired physicals), immunizations record, photo consent, emergency treatment forms, emergency contact information.
Licensor observed last Fire inspection expired 12/27/2025 and provider was not able to provide evidence of most recent Lead. Radon expires 1/8/2028.

Provider reported her liability insurance for her program is up to date however was not able to provider licensor a copy of the liability insurance while licensor was onsite.

Provider reported she had signed in/out sheet posted on the door of her home however the wind blew the sheet away. Provider did have previous day sign in/out sheet.
