An eleven-month-old infant was sleeping on a child’s convertible seat cushion that had been placed on the floor. The provider stated that the child’s parent had given her permission for the infant to sleep in this manner because the child does not sleep in a crib at home and is not accustomed to being in a crib. The provider stated she will request that the parent obtain a letter from the child’s physician, indicating the child’s need to sleep out of a crib during nap time. Meanwhile, she will place the infant in a portable crib for sleep safety.
Breast milk kept in the refrigerator not labeled.
Provider has a verbal feeding plan with parent’s and will request a plan be established and documented in compliance with the regulation.
