A bottle of ibuprofen was observed in kitchen area. Provider stated it came from the child home however it didnt have a name nor proper documentation. Provider asked to discard bottle and not to administer medicine unless having proper documentation.
Assistant present during visit was observed on phone several times when supervising children.
Small items and broken toys were observed to be at easy reach of children. These were removed immediately. No infants were present during visit however there were toddlers present.
Current license not posted.
Medication located in fridge and at a small table in kitchen at easy reach. This was corrected on site.
Mattress had a loose blanket covering it. Removed on site.
All inspections are expired, postings have not been filed out since 2023 or 2024. Workforce registry accounts are not completed. Provider as unable to locate children's files nor staff documentation.

Assistant was observed being impatient with one of the children. Child was having a hard time waiting his turn/ sharing.

Current license has capacity for 8 children however due to NCO's observed during visit and provider unable to provide documentation of 2nd assistant license capacity was reduced to 6 children.
Provider was unable to locate children's files. Stated she has them stored electronically as she is using a new system and is learning how to navigate/work on it. Provider stated she has emergency contacts for parents on her phone and contact info was observed to be posted.
Deck posts needs to be covered as there are more than 3 inches apart and can pose an entrapment issue.
Provider was unable to locate most required documentation (i.e. assistant's files, children's files, etc) Stated she has documentation saved electronically and is still learning how to access it.

Stairs in outdoor play area need to be covered, as well as stairs nearby bathroom that children use. Small area on lower side of the deck needs to be blocked as it has a deep freezer that is not locked, openings that can pose entrapment concerns and a big AC unit.
