Child missing immunizations record
Children were left in program space/room while provider was in bathroom with other child. Children were jumping off couch, climbing on top of table. This also happened when provider was prepping children’s lunch.
Licensor observed little to no age/developmental materials
Fire inspection expired on 11/1/2024 needs to update. Lead expired 5/11/2025 Radon Exp. 3/15/2027
Licensor observed no sense of centers. Provider shared she has changed the layout of her space due to child ripping out posters and destroying toys. Regulation was discussed with provider regarding supervision.

Toxic substances found in the bathroom in reach of children. Medication found on top of table. Licensor removed them during visit and secured them. Licensor inform provider of regulation.
Extension cords observed in program. Provider was advised to remove extension cord as soon as possible.
It was observed that provider had alcohol bottles on side table,in living room. Bottles were behind picture frames but within reach of older children. Provider was asked to place bottles away from children’s reach.
Children have outdated physicals.
