Licensor observed cots to be stacked in a hazardous manner, creating a fall risk if child was to touch the cots.
Garbage receptacle observed to need lid
Upon arrival to the infant classroom, a student was observed to be sleeping in bouncy seat.
Staff observed to be in the bathroom assisting a student washing hands. Due to this the remaining students were out of sight supervision in the classroom.
Allergy posting missing in several classrooms. Allergy list out of date in remaining rooms
Choking poster needed in several classrooms.
Staff were unable to locate the choke prevention gauge. Licensor spoke with administrator regarding regulatory expectations of staff and administration being able to readily locate gauge.
Licensor observed a potted plant on the window sill above cribs. This plant was observed to have low hanging leaves within students reach. Licensor requested the plan be relocated or crib moved away from the wall when used for nap.
upon arrival to infant classroom child was observed finishing their lunch at table without staff within arms reach. Licensor and admin spoke regarding staff being at the table with child. Admin remained at the table with the student while infant teacher assisted licensor
Classroom observed to need lids for receptacles
Licensor observed evacuation crib to need a mattress. As well as items impeding the path for accessibility.
Licensor observed new staff person in toddler classroom. Upon file review program had not received results from DCYF Clearance at the time of the visit.
Licensor observed cots to be torn and in need of repair.
Upon arrival to toddler classroom licensor observed the classroom out of ratio.
Licensing staff observed classroom materials and equipment in need of cleaning. Paint observed to be peeling, chipping and bubbling on walls and window sills. Ceiling tiles observed to have water damage from ice dams
Prek classroom sink not in working condition.
Prek classroom in need of garbage lids on all recepticles
Furniture observed to have staining and in need of cleaning.
Upon arrival to program licensing staff were able to open the door leading into the prek classroom. Additioanlly, staff notified licensor the main enterence was also being worked on as the locking mechanism was not working.
staff file on site did not have comprehensive background check for newly hired staff.

Bedding observed to be in cubbies touching, not stored in individual bags.
Licensor observed medication on site without proper documentation.
Licensor observed cots in need of replacement/ repair. Paint observed in previous visit to be bubbling and peeling continues to be in need of repair. several ceiling lights in need of replacement.
Medication on site observed to not be labeled, missing original packaging, and stored in the incorrect students medication bag. Child with listed allergy without medication on site.
Licensor observed numerous lighting fixtures to be no longer working. Administrator accompanied licensor through the walk and took note of the lights in each room that needed to be replaced.
Cots observed to be torn and in need of repair. Storage container with toys observed to be broken in classroom.

Licensor observed several crib mattresses stored in each crib. The mattresses had a fitted sheet for each child on it. Licensor spoke with admin and staff regarding individual sheets not touching for health and safety and not stacking mattresses in cribs for the children to sleep on top of.
Toddler classroom observed to have bedding left on cots stacked up on one another once children woke up from rest
Evacuation crib observed to be full of classroom supplies
Due to placement of cots and staff students could not be observed by sight supervision during rest.
Medication on site not in original packaging and expired.
Medications in need of medication authorization form and care plans.
Licensor observed cleaning supplies within reach of the children.
