The air filters in the classroom air conditioners contained large amounts of dust. The garbage receptacle in the SA classroom is broken. Ceiling tiles in the Toddler and SA rooms are water stained/broken. A toilet in the shared Toddler/PS bathroom is not working.
When licensors arrived, children were outside playing by age group. The SA section was out of ratio with 1 staff and 16 children
In the PS classroom, the counter area used to prepare/hand out children's lunches was not cleaned/sanitized prior to lunch.

In the PS classroom, staff were not observed sitting with the children during mealtime. In the SA classroom, a child was observed laying on the floor while eating.
The SA bathroom did not contain paper towels. Only one bottle of soap is available for both the Toddler and PS classroom.
In the SA area, two of the mulch beds are worn and one has a large nail protruding out of it. There is a large hole near the fence that needs to be filled. As to outdoor equipment, 3 Little Tykes Cozy Coupes and 1 radio flyer tricycle were broken and need to be removed.
A child who has been enrolled since 2019 does not have any physical exams.
Licensor observed reusable sponges within the classrooms. Provider was informed they cannot be used for cleaning and/or sanitizing and they were removed.
A SA child was observed engaging in screen time while eating. Screen time was via the child's tablet. Tablet was taken away from child while he was eating.
The diaper changing area in the Toddler room contained dirt/sand inconsistent with the time of the visit. Cleaning supplies (dust pan and brush) were also on top of the changing pad.
When licensors arrived, all children were engaging in outdoor play with children separated by age group. It did not appear as if staff were engaging the children in any structured games/activities. Unsafe behaviors were observed in both the SA and PS areas. In the SA area, 2 children were sitting and swinging on the same swing and a child was swinging an empty swing sideways at another child. In the PS area, several children were sitting/standing on the top of the large play structure. It was very crowded and one child sitting with his back at the top of the slide. Staff were not observed consistently redirecting these behaviors, encouraging appropriate behaviors, and/or setting limits.
Sight and sound supervision was not being provided at all times during outdoor play. Children were observed playing a game and were hiding in the area between the ramp and fence. Staff were unaware of the children using this area to hide. Children were also playing in a small section at the end of the ramp putting them out of sight of the staff.
Within the classrooms, licensors several plastic toy bins with cracks. cracked/broken toys, toys with exposed screws and/or nails.
First-aid kits were not outside during outdoor play with a staff also being unaware the kits were required to be outside. First-Aid Kits in all classrooms did not contain the required items and need to be restocked.
Tables in the PS room were not sanitized prior to the children sitting down for lunch.
After coming inside from outdoor play, staff did not wash their hands. Handwashing was also not observed prior to mealtime.
