Three child files were reviewed and all were missing information pertaining to the child's growth and development and parental consent authorizing and/or prohibiting photos/videos.
Six children were in attendance during the visit but only 3 were signed in. Provider reported parents/guardians sometimes forget to sign their child in and/or out. Provider was informed she is able to provide the authorized signature.

Provider's fire inspection expired in December 2025.
Children were playing outdoors at the start of the visit. A table in the outdoor area contained snacks (crackers, etc.) and when the children were eating, a staff member did not sit with the children. Children were observed eating and resuming outdoor play and were not consistently sitting while eating.
Provider's liability insurance expired in November 2025.

In the outdoor space, the following was found to be accessible to children: -One bench of the plastic picnic table is cracked which could cause an injury; firepit contained miscellaneous pieces of wood on the interior and exterior; yard tools (rakes, etc.); storage chest with a broken door with door laying on ground; and debris (bottle caps, cardboard, etc) on grass and pavement
