On 11/5/21 inspection, staff throughout the facility were asked to identify their primary children within the group that they were responsible for. Staff responded that they do not have assigned responsibility for specific child or group. CSRs (Child Supervision Record) were not completed to reflect the staff/child supervision and transition of children throughout the day, staff sign-in and sign-outs were not documented. CSRs for (Discovery) students did not reflect the staff/ student arrival and their transition from center to the transporting School-age staff. Two staff on the outside playground were observed by CR and Director, not supervising their group of children, they were chatting together and were not physically positioned to observe and supervise the children that were actively playing and riding trikes. PQAS-Supervision training was implemented for all staff on 7/27/21 and 10/19/21. On 11/5/21 inspection, the Directors have not ensured that the staff are implementing appropriate supervision of children. In addition, the (CSR-KinderCare) document for ensuring staff/child supervision was not completed and documented as required.
