On 7/24/25, at approximately 12:30pm, a child was observed being supervised by a volunteer (facility person #1) and not a staff person in the infant room. The child was also observed alone and unattended with no staff supervision when the volunteer left the classroom.
Written parental information to be given to parents at the time of enrollment did not include the facility's policies on supervision.
