In the toddler room, there was a Febreze Air Freshener that was plugged into the electrical outlet next to the door and contained air freshener liquid that was accessible to children. In an unlocked drawer in a desk in the front office area, there were multiple adult medications including Ibuprofen and Acetaminophen that were labeled "Keep out of reach of children" and were accessible to children.
On the outdoor play space in the back left corner, there were a couple areas along the fence that had sharp nails and edges that were accessible to children. In the young preschool room, there was a rug on the floor that was not secured to the floor and was a tripping hazard.
On the outdoor play space, the embedded equipment had poles with green paint that were peeling in multiple areas.

There were several fence bolts on the fence facing into the playground that protruded from the fence and did not have covers on the bolts. There was a 4x4 and a 2x4 piece of wood, several feet long, on the ground in the playground and accessible to children, that were both splintered, with multiple wood shards exposed. There were several pieces of garbage on the playground that was not placed in a waste receptacle and was accessible to children.

At approximately 3:50pm, it was observed that more than one the school age child who just entered the facility were in the facility's hallway outside of the office by themselves. Staff person #2, who was responsible for those school age children was standing in the door way of the facility allowing more school age children to enter the building and was not physically present with the school age children who were in the hallway at the other end of the building. At approximately 4:10pm, it was observed that Staff person #1 left the infant room carrying one child and left 3 children alone in the infant room. Staff person #4 was observed to leave the preschool room and enter the hallway three times and thereby left 3 children alone in the preschool room.
A trash receptacle in the Preschool Room containing human secretions was not contained in a closed, plastic-lined receptacle.

The following unsafe conditions were observed in the outdoor play space: a brick, posing a tripping hazard, green, vinyl coated fence tie was unsecured, sticking out and posing an impalement, puncture or cutting hazard. In the young toddler room, a wall vent needed secured properly which poses a pinch point or cutting hazard. In the Preschool Room, an unsecured picture with a glass front was observed posing a cutting hazard. On the window sills in the School-Aged Room, many spider webs were observed. In the Preschool Room, a light fixture above where the children were having circle time, was unclipped and the corner was hanging posing a falling hazard.
In the infant room, next to the window, left of the mirror and towards the bottom of the floor, was peeling paint. In the Preschool Room, yellow peeling paint in the coat rack area was observed. In the young toddlers room, on the back wall peeling paint was observed.
In the Infant Room, rest equipment (cribs) lacked labels with the names of the specific children.

In the boy's restroom, the janitor's closet had toxic materials which were not locked and accessible to children. In the Young Toddler's , a bottle of hand sanitizer and hand lotion, labeled "Keep out of the reach of children" was located on a staff member's desk which was accessible. In the Preschool teacher's upper-right desk drawer was a bottle of perfume, medicated lotion and nail glue which was not locked and accessible to children.
In the Preschool Room, loose screws were observed on a child's wooden chair, at the art table, causing the chair to be unstable posing a fall hazard. In the Preschool Room, a blue, vinyl topped table was ripped with exposed wood and foam padding.

In the outdoor play space the following was observed: A broken plastic, basketball hoop base, multiple broken frisbees and hula hoops. In the Young Toddler room was a broken wooden handle on the play stove under the window. In the Young Toddler room, a yellow "Evenflo" event saucer had a ripped seat cover. A green toy bin on the wooden shelf, in the middle of the Preschool room was cracked posing a pinching or cutting hazard. The toy, play kitchen, top shelf's veneer was lifting which poses a cutting hazard to the children in the preschool room. In the School-Age Room, was an arcade basketball game with a broken, plastic tube posing a cutting hazard. In the School-Aged room a broken, wooden, play stove handle was observed. On both sides of the floor mirror in the back of the School-Aged Room, were two, protruding, two-inch screws, posing an impalement or cutting hazard. In the School-Aged Room's activity table, paper cups were observed to have a black substance which appeared to be mold.
Staff person #3, qualified to be an aide, was observed in the hallway of the facility leading to the outdoor play area supervising a group of children. Staff person #4, who was functioning as an AGS, was taking play equipment from the hallway to the outdoor play space and was not physically present with Staff person #3. Also, Staff person #4 could not see Staff person #3 while taking the play equipment outside. In addition, Staff #3 was observed on several other instances supervising children alone in the infant room and in the hallway while the children were using the restroom.

It was observed that there was a multicolored paper pom-pom accessible to children in the young toddler room. The paper pom-pom, if torn, could produce pieces that are less than 1 inch in diameter and could pose a choking hazard to the children in the room who are still placing objects in their mouths.
