During the time of inspection, it was observed that potting soil and wipes labeled "keep out of reach of children" were accessible to children in the pre k classroom. All toxics were put out of the reach of children during the inspection.
During the time of inspection, it was observed that there was cobwebs in the corner of the room by the train table in the older toddler two classroom. There were ceiling tiles with water marks on them by the wooden beam in the long room. The cobwebs were cleaned during the inspection.
During the time of inspection, it was observed that the container with Mr. Potato head was cracked in the pre k classroom. A shelf in the refrigerator in the pre k classroom was cracked. This was corrected during the inspection.

Observed mulch under 1 inch in diameter throughout the toddler playground. The mulch from the preschool playground shifted into the toddler playground area.
Observed a prescription medication for child #2 which did not have written consent for administration of medication from the parent.

Staff person #3 was observed to be caring for children unsupervised. Staff person #2 who completed the previous health and safety pre-service training modules prior to October 4, 2022, has not completed the 1-hour Pennsylvania Health and Safety Update 2022 prior to December 30, 2022.

On 01/26/2023, staff person #1 left child #1 unattended in the preschool classroom upstairs while transitioning to another classroom downstairs. The child was left alone for approximately 4 minutes until the child's parent found the child crying in the classroom.
Staff person #2 & #3, who completed the previous health and safety pre-service training modules prior to October 4, 2022, did not complete the 1 hour Pennsylvania Health and Safety Update 2022 by December 30, 2022.
Staff person #1 has not completed the following required pre-service training within 90 days of their date of hire (see LIS code sheet): "Get Started with Center-based Care: Building Blocks for Quality". Observed a bottle of clear Gorilla Glue labeled "Keep out of Reach of children" on an open cart next to the teacher's desk which was accessible in the Large Preschool Classroom upstairs.
Observed 2 rolls of packing tape with a sharp metal tear strip on a cart next to the teacher's desk in the Large Preschool classroom upstairs which were accessible to children. Observed charging cords which were plugged into an outlet but not plugged into a device lying on the floor in the Large Preschool classroom upstairs.
Observed medication in the infant classroom which was not refrigerated per the manufacturer's instructions. The medication had been sitting at room temperature from approximately 9:00 am - 12:15 pm until it was discovered that it needed to be refrigerated.
The file for child #1 included a health report which was not updated within 6 months of the previous report for an infant or young toddler (see LIS code sheet). The current health report on file was dated 03/11/2025 with the previous report dated 08/27/2024.
Observed the toilet staff located in the older toddler classroom did not have a hand washing sign.

Observed the nap mats in the PreK classroom upstairs were not labeled. Observed mini Pack N Plays in the infant classroom with regular size Pack N Play sheets on the mattresses in lieu of sheets intended for a mini Pack N Play mattress. The sheets were too big for the mattress and created access material which creates a potential suffocation hazard.
The Child Service Reports for child #1, #2, #3, #4, #5, #6 and #7, providing information to the family about the child's growth and development, were not completed at least once every 6-months. The Child Service Reports for child #1, #2 and #5 were completed in July 2023 and April 2024. Disposable diapers were not being discarded by immediately placing the diaper into a plastic-lined, hands-free covered can. Staff in the older toddler classroom were placing the soiled diaper into a plastic bag before disposing of the diaper in a trash can (which was not hands-free) located across the room from the changing station.
The operator was unable to locate a copy of a current Certificate of Occupancy (COO); the COO on file was a copy of a 90 day temporary permit, which was valid until 06/15/2019.
The file for staff person #3 and #4 included Pediatric First aid & CPR which was not completed within 90 days of hire; the training for staff person #3 and #4 was completed on 12/09/2021.
Observed top connector wires on the fencing around the outdoor play space which were loose. Observed 2 missing fence caps on the fence located in the outdoor play space.

Fire drills conducted between 05/17/2021 - 02/01/2022 were not documented on the fire drill log. Emergency drills were conducted on 07/09/2021 - 09/01/2021 - 01/04/2022 in lieu of a fire drill.
