Based on facility person statements, the following incident occurred on 5/23/23 at 3:20 p.m. Facility person #1 was supervising a group of 3 infants in the infant room when Facility person #1 took the water from a crockpot that was set on high and being used to heat bottles and poured it into a plastic container to sterilize pacifiers. Facility person #1 then placed the container on top of a 27-inch shelving unit and this made the container accessible to a 10-month-old infant who then pulled the container onto themselves resulting in 1st degree burns being sustained from their shoulder blade to their belly and 2nd degree burns being sustained to their right shoulder and a spot directly above their belly button. According to facility person #1, their back was turned away from the infant at the time of the incident, and they did not see how it happened.

The file for Staff Person #2, a rehire, lacked an initial health assessment within 12 months prior to restarting at the facility. The prior assessment was completed 1 year, 11 months, 1 week and 4 days prior to starting. The updated health report was completed 20 days after their rehire.
The mats used for rest time in the downstairs and upstairs main rooms, and the cribs in the Infant Room, were observed to lack labels for whom they were intended.
The file for Child #1 contained consecutive health reports dated more than 12 months apart.
