At approximately 10:05am Staff Person's #1 and #2 were observed to be caring for 15 preschool age children in Classroom 101 (Elephants). Staff Person #1 identified being responsible for 8 of the children present in Classroom 101 and 1 child who was outside the classroom receiving speech therapy. Staff Person #2 reported being responsible for 6 of the children receiving care in Classroom 101. This resulted in one child, in Classroom 101, not being identified by Staff Person #1 or #2. Child #1, who was receiving speech therapy, was not physically present in the classroom with the staff assigned responsibility for the child. The facility lacked signed parental consent from the parent releasing the child to the therapist.

During an interview staff person #1 admitted to having reasonable cause to suspect that a child is a victim of child abuse but did not make a report of suspected child abuse to ChildLine through the hotline at (800) 932-0313, online, or any other method as prescribed by the Department.
On 11/2/2022 while viewing video footage from Friday, October 21, 2022, staff person #1 was observed pulling child #1 by the legs and forcefully dragging him across the carpet. Staff person #1 was also observed grabbing child #2 by her arms and slamming her down in the chair.
