At the time of the inspection in the fireflies infant room a child was in the crib and out of the line of sight of the staff person who was engaged with the other children in their primary care group. On the same day in the outdoor large muscle play space 3 staff persons responsible for the supervision of the children using the embedded play equipment were all on the North side of the unit and the children on the South side were out of the line of sight of the group of staff, who were the only staff in the outdoor play space at that time all of which were observed by this writer as well as the facility's director engaged in their own conversation in a small circle facing each other.

DURING THE RENEWAL INSPECTION, CERTIFICATION REPRESENTATIVE REVIEWED THE EMERGENCY PLAN AND THERE WAS NO INFORMATION PERTAINING TO LOCKDOWN PROCEDURES,SHELTER IN PLACE,SHELTER AWAY AND NO ACCOMODATIONS FOR INFANTS, TODDLERS, CHILDREN WITH DISABILITIES OR CHILDREN WITH CHRONIC MEDICAL CONDITIONS AND THE LOCKDOWN .
DURING THE RENEWAL INSPECTION, CERTIFICATION REPRESENTATIVE CONDUCTED A FACILITY WALKTHROUGH AND INSIDE OF THE DUCKIES ROOM THERE WAS A SWIFFER BROOM NEXT TO THE SINK WITH CLEANING SOLUTION ATTACHED TO THE HANDLE.(CORRECTED ON SITE)
DURING THE RENEWAL INSPECTION, CERTIFICATION REPRESENTATIVE CONDUCTED A FACILITY WALK THROUGH AND IN THE FIREFLIES ROOM, CHILD #1 WAS IN THE CRIB ASLEEP ON THEIR STOMACH.
