3270.54(a) · TIERED LIS: 1. At least two facility persons shall be present in the facility when two or more children are in care. At a minimum, one of the facility persons shall be a staff person. This portion of the plan requires an immediate correction date. 2. For a period of two months, the legal entity/director shall provide a staff schedule on the Friday prior to the following week demonstrating the two staff always being present when two or more children are in care. The correction date for this portion of the plan shall be at minimum two months from the date of the submission of an acceptable plan of correction. 3. Certification Representative will make at least two visits over the next two months to verify compliance. A renewal inspection was conducted at the facility on 3/5/26. Upon arrival at approximately 7:45 AM, only 1 facility person was present with 5 children in care. A second staff person arrived at approximately 8:25 AM.
