Ensure that a Response Action Outcome Letter (RAO) from a Licensed Site Remediation Professional (LSRP) for the site includes the newly added outdoor play area.
Ensure all surfaces are clean and in good repair including ceiling in Room 2.
Provide a barrier, in addition to the fence, for outdoor play areas exposed to vehicular traffic.
Ensure that all rooms approved for use by children are labeled correctly.
Ensure that all staff complete orientation training within two weeks of hire and annually.
Take necessary action to eliminate outdoor hazards including the pothole in the driveway in front of outdoor play area.
Ensure all rooms are approved by the OOL prior to use. Submit a life safety modification request for chapel and kitchen to be approved for use.
Ensure that a minimum temperature of 68 degrees Fahrenheit is maintained in all areas used by the children. At the time of the inspection, temperature in Room 1 was measured at 65 degrees. The center took immediate action and turned on the heat.
