A named staff was aware that a group of children was behind the door she was opening and she failed to ensure that the children were out of the area. A named child's toe was injured by the opening door.
Based on information, retrain all staff on: 1. Maintaining a safe environment, including how to safely enter a classroom occupied by children; 2. The center's policies and procedures for incidents, including completing incident reports accurately; 3. Cooperating with all DCF inspections and investigations, including providing accurate information.
