Based on information, ensure that staff utilize positive methods of guidance and discipline. A named staff grabbed a named child by the arm when the child went to a part of the room they were not supposed to be in. The named child dropped to the floor and the named staff pulled the child up by the arm, causing an injury.
Ensure that the parents, staff and visitors are asked the required questions upon arrival per the Executive Order #149 for reopening.

Ensure that adequate procedures for tracking children are adhered to at all times throughout the center. Staff in room B4 stated they had 14 children but 15 were present.
Ensure that the center complies with the licensed capacity of the classrooms at all times throughout the center. Room B4 had 18 children present when the room capacity is 15.
Ensure that the center complies with the licensed capacity of the classrooms at all times throughout the center. Room B7 had 19 children present while eating snack when the room capacity is 5. Room B4 holds 15 children and 19 children were present in the room.

Ensure that children are supervised at all times including while walking in the hallways/classroom to the bathroom. At the time of the inspection, a staff stood at the doorway leading outside and was watching the class outside and a child was observed going into the classroom to the bathroom unsupervised.
Per updated COVID-19 Standards – Child Care Centers, issued 5.26.2021, ensure children wash their hands with soap and water immediately after use of the bathroom.

Ensure that diapering surfaces are washed and disinfected after each diaper change.
Ensure that toxins are inaccessible to the children at all times. At the time of the inspection, classroom had a Lysol spray at, the sink along with diaper cream and a hand sanitizer.
