Based on information, ensure that adequate supervision is maintained at all times throughout the center, including on the playground. A named child walked out of the playground into the parking lot unsupervised for approximately 6 minutes. Additionally, the 2 named staff were observed with the backs turned to the children while on the playground. 2-24-23: In room 3, a child was climbing on the counter in the classroom while staff was in the bathroom cleaning up.

Staff in rooms 4, 5, and 2 need to ensure that they are using methods of guidance and discipline that are positive. Staff were very loudly, and in angry tone, yelling at children to "go to bed", "sit down", "stop" and "get in line" over and over.
Ensure all staff are retrained from an outside source on positive guidance and discipline including age and developmentally appropriate staff expectations and interactions with children.

Staff did not intervene until the child could be heard crying. 8-7-23: In room 1, a child was observed to be hiding in a toy shelf and another child was swinging around a sticky hand on a string in another child's face and staff did not intervene. 8-7-23: In room 9, a child was stuck in the bathroom and staff did not hear the child trying to get out until this inspector advised staff the child was stuck. 8-17-23: In room 8, a child was observed jumping around on the carpet with a ring pop in their mouth and then sharing the ring pop with another child and staff did not intervene. 8-17-23: In room 1, a child was observed to be sitting on top of a toy shelf.
Ensure that the center's fire protective systems are operative at all times. The center was having work done on the sprinkler system causing the fire panel to be shut down. Abated during the time of inspection. The sprinkler work was completed and the fire panel was back in full operation.
Ensure that food provided by the center is stored, prepared and served in a sanitary manner and food being served is fully cooked. 5-22-23: The center was keeping chicken nuggets which indicated "keep frozen" on the bag in the refrigerator. 5-22-23: Bags of frozen peas also indicating keep frozen were being stored in a milk crate in the hallway. 5-22-23: Large containers of yogurt that were delivered during the time of inspection were left unrefrigerated in a black crate in the kitchen. 5-22-23: Food being made and put on plates for classrooms was not being refrigerated or keep warm and was sitting on the cart for long periods of time before being served. 5-22-23: Whole milk being served to children had a sell by date of 5-17-23. 5-22-23: A storage bin with food and dishes from breakfast was left in room 2 on top of a toy shelf until after lunch was served. 5-22-23: The center was straining apples into a bowl over the sink full of dirty dishes. 7-6-23: Ensure expired food is discarded and food is stored properly. There was expired food in both the refrigerator and freezer and hot dogs that were opened several days ago being stored in an unsealed container.

Work was being done on the center's sprinkler system in room 9, causing the system and fire panel to be in operable while construction was being done.
