RECITE: Ensure that children are directly supervised at all times including when answering the front door, wherein one staff member left her room of 12 children ages 4-5 unsupervised while answering the front door.
Ensure all rooms are approved by the OOL prior to use; make unapproved space inaccessible to children, wherein five children were observed in the center's office, which is not approved space. Additionally, children were observed in the church, located in a separate building, which is not approved center space. Center was unable to provide signed parental permission slips for occasional use of the church.

Ensure fire drills are conducted during each session provided at the center, one fire drill per year is conducted during nap time, if applicable, and all children are evacuated from the building within three minutes during each fire drill. 1/9/26, fire drill log was provided, however, unable to abate as drills were listed over 3 minutes long and number of children in attendance listed far exceeded actual daily attendance. 5/26/26-Note, logs present showed dates of drills from 2024 and one in 3/2026. On a different sheet a drill from 1/2026 was listed. Center instructed that fire drills are required at least once a month.
Ensure that children are directly supervised at all times including during morning drop off hours when the staff member in Room 3 was observed leaving her classroom of 8 children unsupervised to answer the door and walk around the center every time someone arrived. 8/8/25-Unable to abate violation. 9/19/25-Unable to abate violation as a staff member was on her cell phone throughout the inspection while caring for children. 10/29/25-Unable to abate violation as a volunteer member was observed on her cell phone throughout the inspection. Additionally, a child was observed walking back from the bathroom through an office unsupervised.
Ensure that all staff members below 16 years of age are only utilized when school is not in session, or if in session, only when they are participating in a school/work program. Submit documentation for one 15 year old staff member present during school hours. 10/29/25-Note, violation remains open as child under 16 was present during school hours and documentation wasn't received.

Ensure that adequate staff/child ratios are maintained at all times throughout the center, where all three classrooms were out of ratio. Room 1 had 15 children ages 4 and 5 with only one staff member. Room 2 had 13 children (2 children ages 2-2.5 and 11 children 2.5-3) with one staff member and room 3 had 5 children under 18 months with only one staff member. 12/2/25-Unable to abate the violation as staff in Room 2 was observed leaving the room to open the front door, leaving her classroom out of ratio. 3/6/26-Unable to abate the violation as staff in Room 1 was out of ratio and left her classroom to open the door, leaving the room with no staff supervision. 4/1/26-Unable to abate the violation as Room 1 had one staff with 10 4 year olds and 3 school aged children. An additional staff member was required.
Ensure that the director designee does not have full-time classroom responsibilities during the times when the director is not present at the center.

Based on information, retrain all staff on: 1. The center's supervision policy; 2. The center's policies and procedures for tracking children; 3. Maintaining adequate staff/child ratios; 4. The health and safety guidelines outlined by Executive Order 149; 5. The center's discipline policy; 6. Ensuring the playground is free of hazards prior to the children's use.
Based on information, ensure that adequate staff/child ratios are adhered to at all times throughout the center, including on the playground. There were 2 staff with 15 toddlers on the playground when a named child left the playground unnoticed. Staff also leave the classroom out of ratio to assist children in the bathroom and clean the playground. Additionally, staff are left out of ratio during naptime when classes are combined.
Based on information, ensure that adequate supervision is maintained at all times throughout the center, including during naptime, on the playground and during drop off and pick up times. A named child left the playground unsupervised and was found in the road by a passing motorist. Additionally, a named staff was using her cell phone while caring for the children and the staff would leave the children unsupervised while answering the door's during drop off and pick up times. On 6/22/2022, a child was left unsupervised in the bathroom.

Based on information, ensure that staff utilize positive methods of guidance and discipline at all times throughout the center. Infants and toddlers are placed in highchairs to prevent misbehaving and are given 5 minute long timeouts.
Based on information, ensure that the playground is free of hazards, including ensuring that the low hanging wire by the exit sign is removed.

Based on information, ensure that adequate procedures for tracking children are adhered to at all times throughout the center, including on the playground. Two named staff were not aware that a named child had left the playground for approximately 20 minutes. Additionally, staff indicated that they were unaware of the center's policies and procedures for tracking children and had not received training on these polices.
