Maintain all mechanical ventilation and surrounding tiles in a clean condition.
Ensure that all staff who do not have a completed CHRI background check are directly supervised by another staff member. In room A-3, 2 named staff who did not have their CHRI clearances were left alone with a staff, who was not fully cleared, with a group of children.
Complete and submit a DCF Renewal Attestation and all required documents.

A named staff took a picture of themselves with their cellphone while in the children's bathroom and posted it to social media. Two children were in the background of the picture. One child's arm is visible as the child was directly behind the staff, and the second child is not exposed but they are clearly visible with their pants pulled down as they were using the toilet.
2) Ensure children are directly supervised at all times, wherein: a) 4 children, 5-12 years old, were unsupervised on the outdoor play area; b) staff in the fellowship hall were unaware that multiple children were leaving the room to use the hallway bathroom without supervision.
