The program shall request and maintain on file for each child documentation of immunizations in accordance with RSA 141-C:20-a, RSA 141-C:20-b, and He-P 301.14.

The licensing coordinator's review of six children's records revealed there was no updated record of physical examination on file for four children younger than 6 years of age. Corrective action: Our action plan is to get updated physicals for each currently enrolled student, prior to returning for fall programming. In the future, medical forms will be required upon enrollment and prior to attendance. We are at the end of the school year and some families have already had their children out for summer break.
