All center directors, agency administrators, site coordinators, or site directors, and all other child care staff who are responsible for the supervision of children, or who are necessary for the staff to child ratios, shall keep on file documentation of completion of a minimum of 6 hours of professional development, which shall be completed in accordance with the following: (1)  Within 90 days of the first date of employment; (2) Within 2 weeks for programs operating 3 months of the year or less; or (3) By providing documentation of previous completion.

The licensing coordinator's review of three staff files revealed that Staff B, with a hire date of April 2024, had not completed any of the required health and safety trainings, when they are required to be completed within 90 days of employment. Corrective action: Staff Member has continued with the process of completing the health and safety training. All of the training is now complete as many hours of professional training have been added to her staff folder. Future new employees will be required to complete the trainings within 90 days of their date of hire.

Any contaminated, expired, or discontinued medication, whether prescription or over-the-counter, and topical substances shall be returned to the child’s parents or, if belonging to the program, disposed of properly by authorized staff.

The licensing coordinator observed two expired Neosporin ointments in the medical box. Corrective action: The 2 Neosporin ointments were disposed of. The Site Director will check the First Aid Cabiinet monthly to ensure all medications are not expired.

Child care staff shall ensure that for each child, upon the child’s first day in attendance in the program, there is a “Child Care Registration and Emergency Information” (April 2022) form or an equivalent form provided by the child care program completed and signed by the parent, on file, which contains: 1) Full legal name of the child; (2) Child’s date of birth; (3) Child’s physical address and mailing address; (4) Child’s home telephone number; (5) Date of enrollment in the program; (6) The name, physical address, and mailing address of the parent(s) responsible for the child, if different from the child’s address; (7) Telephone numbers for the child’s parents and instructions as to how the parents can be contacted during the hours that the child is at the program; (8) Email addresses for the parents, if available; (9) Names and telephone numbers of at least one person who will assume responsibility for the child, if for any reason, the parents cannot be reached immediately in an emergency; (10) Any chronic conditions, allergies, or medications in case of sudden illness or injury; (11) Written parental permission for first aid treatment; (12) Written parental permission for emergency medical transportation and treatment; (13) The name and telephone number of each child’s physician or health care provider; and (14) Names and telephone numbers of any person(s) other than parents who are authorized to remove the child from the program.

The licensing coordinator's review of eight child files revealed that six of the children’s Registration and Emergency Information forms did not contain a parent signature. Corrective action: The parents of 5 of the 6 children have signed their child’s Registration and Emergency Information forms. One of the 6 children has aged out of our program and no longer attends Sandlot. We transitioned to new digiital forms and I never created a box for parents to sign the registration form. That has been corrected for new registrations.

All staff shall review the program’s EOP within 30 days of their date of hire and sign and date an acknowledgement of their review, with the acknowledgement kept on file for review by department staff.

The licensing coordinator’s review of four staff files revealed that there was no verification of review of the program's emergency operations plan for any staff, who have been employed at the program longer than 30 days. Corrective action: The Emergency Operations Plan has been shared with our Sandown Fire Department and with all staff . All have reviewed the plan and are knowledgeable about how to proceed in any emergency. All newly hired staff will review and sign off within 30 days of employment.

Except for nature-based programs that operate solely outside, programs shall conduct fire drills in accordance with the following: (1) Programs that operate continuously throughout the year shall conduct at least one drill each month of operation; (2) Programs that operate only during the school year shall conduct a drill within 14 days of opening each year and monthly thereafter; (3) Programs that operate only during the summer months shall conduct one fire drill in each month of operation; (4) Child care personnel shall conduct fire drills at varying times during operating hours, including night time hours, if applicable, to ensure that each child attending the program experiences fire drills; (5) Child care staff shall activate the actual fire alarm system for the building for at least 2 of the required monthly fire drills required each year and use a fire alarm or smoke detector to signal all other fire drills; (6) All children and child care staff shall evacuate the building during each fire drill; and (7) Child care staff shall check daily attendance records to ensure that all children and staff are accounted for, after the building is evacuated.

The licensing coordinator’s review of the 2024 written record of fire drills revealed that the program did not conduct a fire drill in January and February, when one drill is required in each month of operation. Corrective action: Sandlot now has a fire drill plan that is easy to follow and we will conduct monthly fire drills as required. A reminder has been set on my cell phone so that the 5th of each month will trigger a fire drill.
