Any contaminated, expired, or discontinued medication, whether prescription or over-the-counter, and topical substances shall be returned to the child’s parents or, if belonging to the program, disposed of properly by authorized staff.

The licensing coordinator observed an Epi-pen in the Pre-K classroom with an expiration date of September 2024. Corrective action: Family notified on 12/12/2024. Parent communicated new epi-pens were ordered. The parent retrieved the expired epi-pen when delivering the new one. The new epi-pen expires 12/01/2025. Going forward, expiration dates of medications will now be listed prevalently on each child's Allergy/medical Action Plan in each classroom. Classroom teachers will notify family one month in advance of expiration date.

A written record of a health screening for all child care staff, household members and other individuals who have 5 or more hours per week of contact with children shall: (1) Be on file at the program and available for review by the department for all child care staff within 60 days of the date of hire or the date the household member or individual begins having daily contact with children; and (2) Include, at a minimum, the information on the “Child Care Personnel Health Form” (11/2017) provided by the department, or its equivalent.

The licensing coordinator's review of five staff files determined that one file did not contain a current health assessment. The previous health assessment expired on June 17, 2024. Corrective action: Staff member was given a health assessment form the afternoon of 12/12/2024 to bring to physician. Prior to reopening following our December break, a list of staff members with the dates of their health assessment expiration will be developed and maintained. Notices of expiration dates will be sent to staff one month prior to the date with a copy of the health assessment form, as done with enrolled children.
