Except for nature-based programs that operate solely outside, programs shall conduct fire drills in accordance with the following: Programs that operate continuously throughout the year shall conduct at least one drill each month of operation.

The licensing coordinator's review of the fire drill log revealed that fire drills were not conducted in December 2024, February 2025 and August 2025. Corrective action: For some reason I thought you had to do 10 drills in a year since they had added on the additional drills you had to do. but I was wrong. I know going forward a fire drill has to be done every month plus 4 additional drills that we will be practicing 1 ever quarter.

A written record of a health screening for all child care staff, household members, and other individuals who work with children in the classroom and who have 5 or more hours per week of contact with children shall: Be on file at the program and available for review by the department within 60 days of the date of hire or the date the household member or individual begins working with children in the classroom for 5 or more hours per week.

The licensing coordinator's review of four staff files revealed that there was no record of a health screening on file for one staff who has been employed longer than 60 days. Corrective action: 1 of them had of gotten missed placed because I know it was in there but that is still my fault for not checking. He has since had his physical and a copy is in, his file. I know going forward I need to review the check list of the required documents for each employee.

In addition to program managers, all child care staff who are responsible for the supervision of children, or who are necessary for the staff to child ratios, shall have on file documentation of completion of training in accordance with (b) below, which shall: Be completed within 90 days of the first date of employment.

Child care staff shall supervise every child in care at all times.

The department received an anonymous complaint that Staff C is leaving children alone in the classroom. Staff A reported that Staff C will leave the classroom to speak with the center director and leave the classroom unsupervised. Staff B stated that Staff C has been spoken to previously about stepping out of the classroom and leaving children unattended. Corrective action: Staff c asks the director to go to her room if she needs to talk to her now. She does not leave the kids unattended she will even line them up and bring them out of the classroom if they need anything.

Programs shall staff group child care centers in accordance with the following: (1) For children ages 36 to 47 months, the maximum group size shall be 24 with the following minimum staffing levels: a. One associate teacher with up to 8 children; b. One associate teacher and one assistant teacher with 9 to 16 children; and c. One lead teacher and 2 assistant teachers with 17 to 24 children; (2) For children ages 48 to 59 months, the maximum group size shall be 24 with the following minimum staffing levels: a. One associate teacher with up to 12 children; and b. One associate teacher and one assistant teacher with 13 to 24 children; and (3) For children ages 60 months and over, the maximum group size shall be 30 with the following minimum staffing levels: a. One associate teacher with up to 15 children; and b. One associate teacher and one assistant teacher with 16 to 30 children.

The department received an anonymous complaint stating that the program was operating out of ratio during morning operating hours. The licensing coordinator reviewed attendance records for various dates in April and May 2025. The review revealed that on April 22, 2025, Staff D was the only staff member signed into the program from 6:11 AM until 7:35 AM. At 7:15 AM, there were 11 children with an average age of 65 months present in the program, which requires a second staff member to be present in the building. On April 23, 2025, Staff D was the only staff member signed into the program from 6:12 AM until 7:33 AM. At 7:15 AM, there were 11 children with an average age of 69 months present in the program, which requires a second staff member to be present in the building. On May 7th, 2025, Staff D was the only staff member signed into the program from 6:15 AM until 7:45 AM. At 7:36 AM, there were 11 children with an average age of 71 months present in the program, which requires a second staff member to be present in the building. On May 14, 2025, Staff D was the only staff member signed into the program from 6:15 AM until 7:50 AM. At 7:15 AM, there were 11 children with an average age of 69 months present in the program, which requires a second staff member to be present in the building. At 7:35 AM, there were 17 children with an average age of 69 months signed into the program, which requires a minimum staffing ratio of one associate teacher and one assistant teacher. On May 19, 2025, Staff D was the only staff member signed into the program from 6:15 AM until 7:50 AM. At 7:20 AM, there were 11 children with an average age of 71 months present in the program, which requires a second staff member to be present in the building. At 7:42 AM, there were 18 children with an average age of 64 months in the program with one staff member, which requires a minimum staffing ratio of one associate teacher and one assistant teacher. In each instance as described, the program was operating without a second staff member when a second staff member was required. Corrective action: I have adjusted the hour for a couple employee, so we are not out of ratio in the morning. My second person is coming in early. I am also waiting for some background checks to clear, so I have extra staff, so this doesn't happen. I have been trying to over staff because you never know when you will lose someone or if something happens. Added pursuant to email on 7/2/2025: Now and in the future the program will be compliant with ratio requirements.

The program shall request and maintain on file for each child documentation of immunizations in accordance with RSA 141-C:20-a, RSA 141-C:20-b, and He-P 301.14.

The licensing coordinator's review of 10 child files revealed that two files did not contain immunization records and three files did not contain a copy of a physical examination. Corrective action: We were going though the file to check on the children's forms and had made a list and copies of new form for the parent to get Physical. We know going forward we need to be checking them every other month to see who need updating. We know the children need new physical for every year for a preschooler and update shoot list for all kids enrolled.

Child care staff shall ensure that for each child, upon the child’s first day in attendance in the program, there is a “Child Care Registration and Emergency Information” (April 2022) form or an equivalent form provided by the child care program completed and signed by the parent, on file, which contains: 1) Full legal name of the child; (2) Child’s date of birth; (3) Child’s physical address and mailing address; (4) Child’s home telephone number; (5) Date of enrollment in the program; (6) The name, physical address, and mailing address of the parent(s) responsible for the child, if different from the child’s address; (7) Telephone numbers for the child’s parents and instructions as to how the parents can be contacted during the hours that the child is at the program; (8) Email addresses for the parents, if available; (9) Names and telephone numbers of at least one person who will assume responsibility for the child, if for any reason, the parents cannot be reached immediately in an emergency; (10) Any chronic conditions, allergies, or medications in case of sudden illness or injury; (11) Written parental permission for first aid treatment; (12) Written parental permission for emergency medical transportation and treatment; (13) The name and telephone number of each child’s physician or health care provider; and (14) Names and telephone numbers of any person(s) other than parents who are authorized to remove the child from the program.
