The center director, site director, family child care provider, and all staff used to meet staff to child ratios shall: (1) Be certified in pediatric cardiopulmonary resuscitation (CPR) and first aid within 90 days of the first date of employment; (2) Obtain certification in (m)(1) above by the American Red Cross, American Heart Association, Emergency Care and Safety Institute, National Safety Council, or other nationally recognized organization; and (3) Maintain current certifications required in (m)(1) above.

REPEAT CITATION PREVIOUSLY CITED ON SEPTEMBER 13, 2022. The licensing coordinator’s review of five staff records revealed that one staff, who has been employed at the program for more than 90 days and who is used to meet staff-to-child ratios, was not currently certified in First Aid and CPR. This constitutes the first repeat citation and failure to comply with one approved CAP regarding staff’s CPR and First Aid certification requirements. Corrective action: Moving forward all will take the required CPR training with the 30 days of employment. I have had the staff take the required CPR training and have an appointment set up for the physical CPR test done this week.

In center-based programs, staff shall position themselves to: (1) Be able to hear all children younger than school age at all times, continuously scan the entire environment to know where children are and what they are doing, and be able to physically respond immediately; (2) Know the whereabouts and activities of all school age children in their care at all times when children are briefly out of sight, such as when transitioning from one area to another or using the bathroom, and shall be able to physically respond immediately; (3) Allow for visual supervision of all children while children are eating, and shall be able to physically respond immediately to any child that may be choking; and (4) Know the number of and identity of children in their care.

The licensing coordinator observed that 14 children were present on the day of the visit. Staff A reported that there were 15 children present, and Staff B was unaware of how many children were present. Corrective action: Moving forward all staff will know how many children are present in their care at all times, and count children every couple of minutes. The number of children will be displayed in the room where all staff can see it. I have staff counting the children when the leave a room, enter a room or are in line.

A written record of a health screening for all child care staff, household members and other individuals who have 5 or more hours per week of contact with children shall: (1) Be on file at the program and available for review by the department for all child care staff within 60 days of the date of hire or the date the household member or individual begins having daily contact with children; and (2) Include, at a minimum, the information on the “Child Care Personnel Health Form” (11/2017) provided by the department, or its equivalent.

REPEAT CITATION PREVIOUSLY CITED ON SEPTEMBER 13, 2022, AND DECEMBER 14, 2021. The licensing coordinator’s review of five staff records revealed there was no health screening on file for three staff. This constitutes a second repeat citation and failure to comply with two approved Corrective Action Plans regarding staff health screenings being on file. Corrective action: Moving forward all staff health screening forms will be in every staff file and be up to date. This is put in our new hire packet.

All staff shall review the program’s EOP within 30 days of their date of hire and sign and date an acknowledgement of their review, with the acknowledgement kept on file for review by department staff.

REPEAT CITATION PREVIOUSLY CITED ON SEPTEMBER 13, 2022. The licensing coordinator’s review of five staff records revealed two staff did not have documentation on file acknowledging that they reviewed the program’s Emergency Operation Plan within 30 days of their date of hire. This constitutes a repeat citation and failure to comply with one approved Corrective Action Plan regarding staff's review of the EOP. Corrective action: Moving forward every staff member will have a signed copy of the EOP showing that they have read and understand the EOP. We have set quarterly meetings to go over the EOP and have them signed.
