If any child while in the care of the program sustains a serious injury, as defined in He-C 4002.01(bk), or otherwise requires emergency services as prescribed in first aid and CPR training, the program manager or designee shall: Notify the department within 48 hours via NHCIS.
During the review of an injury report submitted by the program staff, the licensing coordinator noted that the report was submitted four days after the incident occurred, exceeding the 48-hour reporting requirement. The Center Director confirmed that they had forgotten to submit the report within 48 hours. Corrective action: Action Plan: All injury reports will be submitted within the required 48-hour timeframe moving forward. The Center Director will implement a tracking system and checklist to ensure timely submission. Administrative staff will be re-trained on licensing reporting requirements. A Child Illness and Accident Binder has been created for staff reference, which includes organized guidance, required forms, licensing reporting guidelines for all incidents, and resources to support identification and prevention of illness and accidents. This plan was implemented and all resources are live and ready for use.
