Prior to employment or residency, as applicable, and every 5 years thereafter, all child care staff, substitutes, other employees, and volunteers who may be alone with children or are included in staff to child ratios, and household members age 10 years or older, shall submit for a background record check using NH Connections.

The licensing coordinator reviewed staff attendance records and determined that one staff did not submit a complete background record check prior to employment. The director confirmed this individual did not submit for a background record check prior to employment. Corrective action: All employees after getting fingerprinted create their NH connection page, to conduct all the NH background check . This employee was sent for another fingerprinted session on Tuesday 10/22 at 9:30 am. She will not be at the property until deemed eligible .

Garbage shall be disposed of in a lined and covered container and staff shall empty trash containers daily or sooner if contents create an odor or a health risk.

The licensing coordinator observed that there was one uncovered trash can in the Multi Age Room-A classroom. Corrective action: Teacher was reminded to use trash with the lid. As there was a barrel in the classroom with a lid. Teachers have always taken the trash bag out right after meals. Teacher now uses the barrell with lid for all food products and still takes trash out after meals.

Breast milk and prepared formula shall be stored in covered containers, labeled with the child’s name and, dated.

The licensing coordinator observed three prepared bottles in the Infant C classroom that did not have the date written on them. Corrective action: The teacher put the date on it right away and reminded parents that all bottles must be dated. All bottles are double checked at drop off for dates. If bottle does not have a date, parents write it right way.

Programs shall have available for review by the department and parents a written schedule or plan which details the daily activities offered to children.

The complainant stated that an older, bigger child picked up a young child and dropped them, resulting in the younger child hitting their head on the concrete. The licensing coordinator interviewed staff, reviewed staff statements and the injury report, all of which revealed the staff did not protect younger children from injury caused by older, bigger children. Corrective action: Staff has been reminded and retrained that children are not allowed to pick up any children.

If any child has a serious injury while in the care of the program, consisting of fractures, dislocations, stitches, second or third degree burns, concussions, loss of consciousness, or requires emergency medical treatment or hospitalization, the family child care provider, center director, or site director shall (1) Immediately notify emergency personnel and the child’s parents; (2) Notify the department within 48 hours viaj e-mail, fax, or the NH Connections portal; and (3) Within 7 days, provide the department a written report via e-mail, fax, or the NH Connections portal, detailing the nature and circumstances of the serious injury.

The complainant stated that they were not notified of their child's serious injury until they picked up at about 5:20pm. Staff confirmed that the family was not notified of injury until pick up. The program also failed to notify CCLU of the serious injury within 48 hours and did not provide any report or documentation within the seven day period. Corrective action: Staff has been retrained to make sure to notify about all injuries that are severe or shoulders and above to families right away. They will call and also send picture and message in the app to families. Director or Assistant will notify CCLU within the 48 hour window and also make sure to email over documentation within the 7 day period.

In center-based programs, staff shall position themselves to: (1) Be able to hear all children younger than school age at all times, continuously scan the entire environment to know where children are and what they are doing, and be able to physically respond immediately; (2) Know the whereabouts and activities of all school age children in their care at all times when children are briefly out of sight, such as when transitioning from one area to another or using the bathroom, and shall be able to physically respond immediately; (3) Allow for visual supervision of all children while children are eating, and shall be able to physically respond immediately to any child that may be choking; and (4) Know the number of and identity of children in their care.

The program self-reported a child being left in a classroom for approximately 30 minutes without supervision. Staff B and Staff C reported that they counted the child in the name-to-face check before leaving the room for an in-house field trip. Staff B noticed the child was not with them outside, and Staff A found the child in the classroom after 30 minutes had elapsed. Staff D reported the incident to CCLU, and has provided remedial training to Staff B and Staff C. Corrective action: Staff B and C were retrained on name to face checks, and a 3rd person has been added for the next 30 days to supervise all transitions that are done in the class and every threshold. We have retrained staff and it was also addressed at our staff meeting to all of our staff as a refresher. ALL of our staff know to check attendance with each child before leaving the classrooms.

The licensing coordinator observed a can of shaving cream on a low bookshelf in the multi-age classroom that was accessible to children. Corrective action: This has been removed and placed higher where children do not have access. Shaving cream will no longer be used in our center, we will use a different sensory item. Teachers will be in direct supervision of any sensory activities.

Sinks that are used for food preparation or clean up, including sinks used for getting water for baby bottles, rinsing bottles, or dishes, and washing toys, shall not be used for hand washing after toileting or diaper changing.

The licensing coordinator observed a staff in the multi-age classroom use the hand washing sink to fill a water bottle for drinking. Corrective action: Teacher was a new hire and she has been reminded that it is a hand washing sink and will not use it again for drinking. During our onboarding process, we will ensure that all new hire's know that they cannot use handwashing sinks for drinking.
