Child care staff who have not yet completed the training in (1) through (3) below shall work under the direct supervision and observation of a staff member who has completed the following trainings: (1) Prevention, recognition, and reporting of child abuse and neglect; (2) Prevention of shaken baby syndrome and abusive head trauma; and (3) Pediatric first aid and pediatric CPR.

The licensing coordinator’s review of four staff files present on October 9, 2025, who consistently work in the program, revealed that none of the staff had completed training in prevention of abusive head trauma. Corrective action: All staff will complete the Abusive Head Trauma/ SIDS training within the ProSolutions NH State Health and Safety Trainings. Staff will be required to submit the certificate of completion to the Registrar, which will then be housed in their online staff file and a copy will be put in their paper file on site. A deadline of October 24, 2025, has been set and communicated to all staff still needing to complete the training. In the future, this training will be assigned during the staff training prior to the start of the school year.

Program managers shall ensure that for each child there is a “Child Care Registration and Emergency Information” (August 2025) form, completed by the parent or guardian, or an equivalent form on file on the child’s first day in attendance.

The licensing coordinator's review of 10 child files revealed that none of the children's registration and emergency information forms were the equivalent of the August 2025 form, as the forms were missing the required statement under He-C 4002.10(c). Corrective action: A checklist has been generated for all registered students so that Site Directors can check off each form as they are signed by the parents/ guardians. Once the forms are signed, they will be kept in the student's files on site. All newly registered children will receive a form on their first day of attendance for their parent or guardian to sign. A deadline of October 24, 2025, has been set and communicated to all site directors regarding the completion of these forms. These forms will remain on file with new ones issued to students who are new to the program each year.
