Authorized staff shall complete training in medication safety and administration every year.

The licensing coordinator's review of five staff files indicated that none of the personnel authorized to administer medications received training in 2025, which is required to be completed annually. Corrective action: I was unaware that there was a new rule change in regards to the "Administering Medication" needing to be done yearly, or I would have definitely had myself and all staff take the training again. The day licensing came in and told me about this rule change, I went home and retook the course, as well as two of my FT staff. Certificates were put in files on June 28th, 2026. The two PT staff that will be returning next year, are currently working on redoing theirs. All certificates will be in files by June 12th, 2026. Moving forward, I now know this new rule and will have all staff, including myself, retake this yearly. This will be added to the "check list" in everyone's files.

For programs that administer medication: Programs shall not accept any prescription medications that do not include a prescription label or medication order from a licensed practitioner.

The licensing coordinator observed that one child's inhaler at the program did not have a prescription label. Corrective action: Parent accidently took box home with them when administering their child's inhaler before leaving the program and forgot to bring it back. Parent removed inhaler because there's no need more it during the summer and because school is almost over. Moving forward, if parents ask to administer their child's medication from the program before leaving, I will make sure they return the medication in it's original box with the label on it before putting medication back in the box. I have sent out an email to all staff asking the medication section of the rules has been reviewed. This ensures if I'm out and another staff accepts medication they will be aware of what can be locked up and what they can accept. All inhalers and EpiPens will be stored in our emergency bag, high and out of reach for all children.

Child care staff shall ensure that for each child, upon the child’s first day in attendance in the program, there is a “Child Care Registration and Emergency Information” (April 2022) form or an equivalent form provided by the child care program completed and signed by the parent, on file, which contains: 1) Full legal name of the child; (2) Child’s date of birth; (3) Child’s physical address and mailing address; (4) Child’s home telephone number; (5) Date of enrollment in the program; (6) The name, physical address, and mailing address of the parent(s) responsible for the child, if different from the child’s address; (7) Telephone numbers for the child’s parents and instructions as to how the parents can be contacted during the hours that the child is at the program; (8) Email addresses for the parents, if available; (9) Names and telephone numbers of at least one person who will assume responsibility for the child, if for any reason, the parents cannot be reached immediately in an emergency; (10) Any chronic conditions, allergies, or medications in case of sudden illness or injury; (11) Written parental permission for first aid treatment; (12) Written parental permission for emergency medical transportation and treatment; (13) The name and telephone number of each child’s physician or health care provider; and (14) Names and telephone numbers of any person(s) other than parents who are authorized to remove the child from the program.

The licensing coordinator's review of 10 child files revealed that 10 of the children's registration and emergency information forms were not the equivalent of the April 2022 form, as the forms were missing the required statement under He-C 4002.10(c), and two of the forms on file had not been updated annually. Corrective action: I went to the licensing site and downloaded the updated emergency form. This was sent out to all parents the week of Oct. 2nd 2023. I now have ALL emergency forms filled out and back in files. Moving forward, I will make sure we always have a copy of any updated registration forms so that this wont be an issue again. I will make sure to constantly check for updates on the Licensing website.
