Child care staff shall supervise every child in care at all times.

Program staff self-reported an incident involving a lack of supervision. Staff A, B, and C confirmed that a child was left unsupervised inside a bathroom in the program for three to five minutes while their class transitioned to the outdoor play area. Child A was discovered by Staff B in the bathroom of the classroom as Staff B was returning inside with another child who needed to use the bathroom. Child A was reunited with their class and program staff immediately contacted Child A's parent to alert them to the incident. Corrective action: We have added a policy to the employee job description for all assistant, associate and lead teachers to mandate Name to Face transition safety protocol. On the app “ProCare” has the name to face safety feature to ensure the attendance is accurate to the children in our care, and each child is accounted for. We also changed our policy to allow parents to only sign their child in/out on the QR code outside of the classroom, rather than in the parking lot, to avoid a lag in attendance and ensure accuracy. All staff will read over the new policy, and sign to acknowledge. Staff will read over the policy change and sign to acknowledge.
