The program shall maintain on file for each staff member, whether paper or electronic: Documentation of the staff’s qualifications, including required work experience and education, which demonstrates that they meet the qualifications for their position.

The licensing coordinator's review of five staff files revealed that one file did not contain documentation of the staff member's qualifications, including but not limited to a resume and transcripts. Corrective action: Attached are the diplomas for the staff member that did not have documentation. They have also been placed in the staff's physical folder. All staff member folders will be checked for completion of this documentation. The new employee checklist has been updated and is what is shared with all new hires to ensure that all documentation is complete upon hire.

All prescription or over-the-counter medication shall be kept in the original containers or pharmacy packaging and properly closed after each use.

The licensing coordinator observed one inhaler in the West Classroom that was not in the original pharmacy packaging. Corrective action: The prescription with the child's name, medicine name, and dosage was acquired from the family by the end of the day on October 17th and placed with the inhaler. We have highlighted the requirement for original prescription with name that is on the Medication form that each family completes when bringing in medication.

A written record of a health screening for all child care staff, household members, and other individuals who work with children in the classroom and who have 5 or more hours per week of contact with children shall: Be on file at the program and available for review by the department within 60 days of the date of hire or the date the household member or individual begins working with children in the classroom for 5 or more hours per week.

Prior to employment or residency, as applicable, and every 5 years thereafter, all child care staff, substitutes, other employees, and volunteers who may be alone with children or are included in staff to child ratios, and household members age 10 years or older, shall submit for a background record check using NH Connections.

The licensing coordinator observed one staff in the program whose background record check was not submitted prior to employment. Corrective action: The director immediately shared the appropriate steps to be taken with the staff member. The background record check was completed on Monday 3/25/24. The director has added the guide (pdf) of steps to complete background checks to the new employee guide and will share it with future new hires to ensure that similar issues will not happen in the future.

The program shall maintain daily child attendance records, in either paper or electronic format, including the child’s full name, date of birth, and their arrival and departure times in the program and each room or group throughout the day, as applicable, recorded in real time by staff or parents.

The licensing coordinator's review of program records revealed child attendance records in the East Classroom were only completed with arrival times of the children in the classroom. One child had left the program and was not signed out of the classroom on the day of the inspection. In the West Classroom, all children were signed out of the classroom, prior to their actual departure. The review of staff attendance revealed two staff members were signed into the program, when there were eight staff in attendance on the day of the inspection. Staff attendance records did not include the staff member’s scheduled work hours. Corrective action: On March 21st, during the staff meeting, the director shared the changes that needed to be implemented in attendance records for both classrooms and for staff. We have already made those changes - accurate recording of when children arrive and leave, after they arrive or leave. Staff attendance has been changed to be on the children's attendance sheet to ensure timely check-in/check-out. We will continue to use this method in the future.
