The licensing coordinator observed one staff member, Staff A, working in the program whose background record check was not submitted prior to employment. Corrective action: Have confirmed all staff is now linked to our program. Background checks have been completed for Leah, and Amanda. Regional Director will be following up with with any new hires, and ask for proof of the process where they have completed their fingerprints, and initiated background check through the NHCIS Portal. Director will follow up in NH Connections assure the employee is linked to program and has successfully passed their background check before starting in the program.

The program manager shall update the staff roster in the NHCIS portal within 5 business days when the individuals as described in (b) above are no longer working in the program or part of the household, with the date they left the program.

A review of the staff roster in NHCIS revealed that it was not current because it included the name of one staff member who is not working at the program.
